CSWU Parent Travel Consent Form
Parent travel consent form is for CSWU players traveling without parents.
To whom it may concern
This letter is to confirm that the undersigned are the parents/guardians of the player that we do hear by give permission for the designated guardian to act as guardian(s) for the player during the designated period that the player is traveling to/from and participating in the designated tournament with the designated team of the Calgary Southwest United soccer club.
The guardian(s) have the authority to approve emergency medical procedures in the event that the parents cannot be contacted.
The team will be leaving Calgary for the designated destination travel area on the designated dates and returning to Calgary on the designated dates and as such will define the period and travel area of authorized guardianship.
